By completing the form below, you are indicating your intention to participate in this campaign. The information you submit in this form will be shared with The Canadian Shaare Zedek Hospital Foundation so that you can receive a donation receipt, where applicable, and to the extent permitted by law.

After you submit the completed form, an Israel Bonds representative will call you to complete your donation of Israel bonds to The Canadian Shaare Zedek Hospital Foundation.

Help Giraffa, our campaign mascot… with your generosity, her spots will appear and she will grow!

By participating in this special campaign:

	You will be able to purchase Israel bonds for The Canadian Shaare Zedek Hospital Foundation
	When the Israel bond matures in one year, The Canadian Shaare Zedek Hospital Foundation will redeem it, receiving principal plus interest at maturity, and put the funds towards the purchase of a Giraffe Incubator Carestation
	As a donor, you will receive a donation receipt from The Canadian Shaare Zedek Hospital Foundation, where applicable (and to the extent permitted by law)

By participating in this special campaign:
